Important Information for Incoming 7th Graders

Attention Kenton City Schools’ 6th Grade Parents/Guardians:

You may not be aware that the Ohio Department of Health’s requirements for entry into 7th grade requires all 7th graders to obtain the DTaP booster (Tetanus, Diphtheria, and Pertussis) vaccine prior to the start of school August 2015.  This is an ongoing requirement for entry into 7th grade.  If your child has not had this required vaccine or you have not completed and returned a declination form as a waiver for the child not receiving this vaccine, the child will not be allowed to attend school until one of these processes is completed.

The process for obtaining this vaccine:

  • Contact your child’s health care provider for an appointment for this vaccine, and then bring proof into the Kenton Middle School front office.
  • Call the Kenton Hardin Health Department to make an appointment for the shot clinic at 419-673-6230, option 4.

In order for your child to obtain the vaccine(s) the parent or legal guardian must sign for written permission for this to be given to your child by the Kenton Hardin Health Department, if the parent will not be bringing the child to the health department for this vaccine and permission for the adult bringing the child to the health department for this purpose.

To assist parents in meeting this requirement, the Kenton Hardin Health Department offers its usual Thursday vaccine clinic from 8:30 AM – 12:00 PM and 12:30 PM – 4:00 PM.  Their address is 175 West Franklin Street, Suite 120, Kenton, Ohio.  Their phone number is 419-673-6230, option 4.  You must call and make an appointment!

The Kenton Hardin Health Department will also be available to administer these vaccines at the Kenton Middle School in their mobile van, which will  be parked in the parking lot at the back of the middle school the day the incoming 7th graders come to pick up their schedules late this summer.

If your child has a contraindication to having this required vaccine due to health conditions, medical conditions, or philosophical reasons, MUST complete, date, and sign a Kenton City Schools Waiver of Vaccines form that is available at the Kenton Middle School and Kenton Elementary School offices.  This must be on file at your child’s school office before the first day of school in August 2015.
